We collect personal information directly from you or your authorized representatives during assessments, consultations, or through forms. With your consent, we may also collect info from government entities (like My Aged Care) or your healthcare practitioners.

We only use your personal and health information to manage and deliver your support services, process payments, and ensure regulatory quality standards are met. We may disclose your information to:

  • Your nominated representatives or family members.
  • Your doctors, allied health professionals, or hospital emergency teams in urgent situations.
  • Government bodies (like the Aged Care Quality and Safety Commission or Department of Health) as required by funding agreements and safety laws.
